Offers a consistent and efficient way to collect assessment information, review and manage risks, and measure outcomes.
Developed in partnership with a well-established statistical research programme based at University College London.
Flexible to any setting
Record risk factors and warning signs most commonly evidenced in each specific care group. Includes the opportunity to add any other risk factors relevant to the individual.
Multi-layered risk assessment
Each risk tool has been carefully designed to allow professional judgement to be formed and recorded within a systematic framework. A 'risk score' is arrived at for the main domains of risk for the specific client group, allowing change over time to be easily tracked.
The FACE Risk tools facilitate high quality, proportionate assessment of risk with integrated risk management planning. The suite contains a set of evidence-based, multi-professional and multi-contextual tools. The development of each tool has drawn on the expertise of senior clinical staff from around the country and supports national policy guidelines.
FACE Assessment Toolsets are a product of over 20 years' development work involving hundreds of practitioners, service users and carers. They are designed to meet key clinical, management and national requirements.
They are also updated regularly with the involvement of our professional user network. We are constantly seeking to both improve existing tools and design new tools to meet emerging needs.

Training and support
All toolsets include high quality supporting documentation and training materials. We can provide on-site training for new users through our network of experienced trainers.
In addition, we run a training programme for people who wish to become 'expert users'. This course is designed for senior practitioners or trainers who will be taking a lead in implementing FACE toolsets in their organisation.
