Overview

The FACE Perinatal Assessment Toolset offers a comprehensive documentation system for the routine recording of clinical data and the measurement of outcomes in perinatal mental health services.

It is designed to meet the need for robust measures of clinical outcome, patient-reported outcomes and measures of quality and performance such as CQIN, as well as broader national requirements of CPA, and the mental health minimum dataset.

The toolset has been developed collaboratively with East Midlands Specialised Commissioning Group, East Midlands Perinatal Clinical Network, and Nottinghamshire and Derbyshire Mental Health Trusts.

FACE Assessment Toolsets are a product of over 20 years’ development work involving hundreds of practitioners, service users and carers. They are guaranteed to meet all clinical, management and national requirements.

They are also updated regularly with the involvement of our user network. We are constantly seeking to both improve existing tools and design new tools to meet emerging needs.

Included in Toolset

  • Background Information
  • Perinatal Overview Assessment
  • Risk Profile
  • Your Treatment and Care Assessment
  • Patient Reported Outcome Measures (PROMs): 'How are you?' Assessment

Training and Support

All toolsets include high quality supporting documentation and training materials. We can provide on-site training for new users through our network of experienced trainers.

In addition, we run a central training programme for people who wish to become ‘expert users’. This course is designed for senior practitioners or trainers who will be taking a lead in implementing FACE toolsets in their organisation.
